Dear Mr. Henry,

It has been noticed that despite giving you multiple verbal warnings you have been using your mobile phones frequently during the office hours. The department has received lots of complaints about you talking on the mobile phone throughout the office hours and speaking loudly on it which has been disturbing your colleagues.

Your manager, Mr. William Ray, has noticed you multiple times and has warned you so many times to reduce the usage of mobile phones at office as well as be soft while talking on the phone but you don’t seem to have paid attention to any of these warnings.
