Dear Mr. Myers,

Please accept this letter to express our deep condolences on the sudden death of your father on 06th of this month. We understand the phase you are going through at present and we really didn’t want to disturb you, but there are few formalities that need to be completed after the death of your father, who was an account holder at our Bank.

According to our records, your father had kept you as the nominee of his account with us. We are requesting you to please fill in the attached form so that it becomes easy for us to transfer the money from your father’s account to yours. We understand your situation at this point of time, but like I said above that this is a mandatory formality that needs to be completed as soon as possible.

We again express our condolences for your loss and thank you in advance for your understanding regarding this matter.

Sincerely,

Joseph Stevens
