Dear Ms. Brown,

I, David Saunders, am writing you this letter to get medical authorization from Nice Buy Pvt. Ltd as I am working here as Marketing Manager in the Marketing Department since April 20th, 2007. When I joined, I was given assurance by the HR department of getting medical authorization from company underneath I can use certain medical services and facilities. I have attached a copy of joining letter for your reference.

Now that I have completed my term of three years with this company and according to legal clause, I am eligible for medical authorization. Please let me know the system to complete this authorization and what all documents are required for the same. As informed by HR department, I have already submitted my medical records and my latest salary slip.
